Subscription Services and Terms

1. As a subscriber, my company is entitled to borrow up to three audio visual programs at a time for up to two weeks, or four weeks depending on membership chosen, without any rental fee.
2. The subscription is for a period of one year from the month the Safety Video Library application and fee is received by the National Resource Safety Center.
3. As a subscriber, my company will receive discounts on many items that may be purchased from the National Safety Center. Discounts include moderate quantities of audio-visual programs.
4. As a subscriber, my company agrees to pay any shipping and handling or late charges associated with the loan of the visual materials.
5. As a subscriber, my company agrees to adhere to the audio visual loan terms.

MEMBERSHIP SERVICES
The audio-visual programs in our library are produced by the nations leading safety program producers and are available for free use to all members. A maximum of three programs may be checked out without incurring any rental fees. If the programs are not reserved by another company, the time may be extended for a $29.00 per week service fee by calling our Library PRIOR to the programs due date. The member is responsible for shipping and handling charges, late charges and charges associated with damaged or lost programs.

LATE CHARGES
Any program borrowed that is not in our office by 5:00pm on the due date will be considered late and subject to a maximum late charge of $15.00 per day, per program borrowed. Any audio-visual program received in our office after the due date will be considered late, and a fee shall be assessed for each day the program is late, up to the maximum charge of twice (2x) the full replacement cost of the program. To avoid any late charges, return all items borrowed before the due date.

SHIPPING AND RETURNING OF AUDIO-VISUAL MATERIALS
All programs will be shipped via an express delivery service or they may be picked up at our office. Members are requested to return programs via an express delivery service that will provide full insurance for the value of the program(s), or they may be returned personally to our office. Each audio-visual program must be insured for $400.00 each or the member assumes full responsibility for the replacement cost of lost or damaged programs. Members assume full responsibility for all videos shipped or picked up by other companies authorized by members. Please contact or office if programs are going to be returned late so that an adjustment in the schedule can be made. Continuous failure to return programs on time may result in revocation of membership services.

RESTRICTIONS
Programs may not be transferred from one organization to another, nor may these programs be shown where an admission is charged.

NOTE: Federal law provides severe civil and criminal penalties for the unauthorized reproduction, distribution, or exhibition of copyrighted motion pictures. (Title 17, United States Code, Sections 501 and 506)

RESERVATIONS
Reservations may be made up to 60 days in advance. Since shipping and handling time must be considered, as well as the actual time the audio-visual programs are in your possession, we limit your advance reservation to three (3) programs per month. Audio-visual programs may be previewed at out office only. An appointment must be prescheduled with our Librarian.

Library hours are from 7:00am to 5:00pm, Monday through Friday, Pacific Standard Time.

INVOICES
Members will be invoiced for insured shipping and handling charges, extended loan periods, late charges, and lost or damaged audio-visual programs. Terms are net 30 days from the date of invoice. Failure to pay invoices in a timely manner may suspend your membership services.

DAMAGED AUDIO-VISUAL PROGRAMS
Members are liable for lost or damaged programs. Damage to programs will be charged at the rate of $60.00 per hour with a one (1) hour minimum. Members will be held liable for twice the full replacement costs of programs in the event of their loss or severe damage.

The terms of this agreement are subject to change without notice.
